We accept all insurance's except Medicaid Patient 1st and HealthSpring.
If you are unable to keep your appointment we ask that you provide us with a courtesy call as soon as you are made aware. We reserve the right to charge a fee if you do not show for your appointment without informing us in a timely manner. After 3 no shows we may ask that you find another primary care provider.
Your co-pay is due at the time of service. If you do not have your co-pay at the time of your appointment, you may be asked to reschedule.
